Our agreement is that 2D is Flannery:
Pard forgets and opens 2D (no alert)
West however asks him what it means waking him up.
So he tells west it means 4s  5h 11-15 hcp  (which i think is correct because that's our agreement)
 
All the other slam bidders were in diamonds so we got an mp top.  Were EW fixed or damaged somehow?

You are allowed to misbid, but as your opponent I would be "not happy" because you are such a longstanding partnership, but that would be my problem.
 
With 20 points and thinking Tom had 11-15 and 4 spades I am surprised you did not double a very vulnerable west.

North certainly does nothing wrong, because he plays by the agreement.  I'm wondering if south since he failed to alert 2D could have a problem. 
 
For example if west says to south "you are supposed to alert flannery"  what's south say?  Lie and say I forgot to alert?.   What if the failure to alert then the subsequent explanation convinces the opps we are in a bidding misunderstanding and dbl getting an even rounder zero?

As the others have said: no score adjustment.

South only has a problem if he receives UI from North that wakes him up. If he is woken up by an opponent, by North's *bid*, or remembers on his own, he has no ethical dilemma, just issues the missed alert and gets on with choosing his next bid.

If EW double because they know NS have had a misunderstanding, they get to enjoy their even rounder zero.
