Alternatives
for Adults with Physical Disabilities

 


Alternatives is a Medicaid Waiver program that will provide home and community-based services to a limited number of adults with physical disabilities.

What services are available?

Alternatives offers two consumer-directed services:

  • Attendant Care: Assistance to accomplish tasks of daily living, based on need and approved by the physician. Based on need, the client may receive up to 8 hours a day, 7 days a week of attendant care. The client shall recruit, hire, supervise and approve payment of the attendant. Although the attendant may be a family member, it may NOT be a spouse or other legally responsible person.
  • Environmental Adaptations: Modifications to the environment that increase independence or accessibility.

Who can apply?

Anyone who:

  • Has a physical disability and income of no more than 300% of SSI

  • Is between 21 and 64 years old

  • Meets eligibility for Intermediate Level Nursing Home Placement
    Note:
    To be eligible for Intermediate level nursing home placement, the individual must require extensive assistance with 1 ADL or limited assistance with 2 ADLs (ADLs considered are transferring/locomotion, eating or toileting), or have a diagnosis of dementia, or have a medical condition that requires daily monitoring by a medical professional. Individuals that required skilled care cannot be served.

  • Has in-home service expenditures of no more than the cost of nursing home placement.

How do I apply?

Contact the Department of Human Services Office in the county of residence to apply for Alternatives. An Alternatives Counselor will arrange to visit in the home to complete an assessment, which must be approved by your physician.

What is the role of the Alternatives Counselor?

The Alternatives Counselor will:

  • Complete an evaluation that will be the basis for determining eligibility;

  • Develop a plan of care based on the needs and wishes of the client;

  • Assist in enrolling providers of choice;

  • Complete an annual reassessment of needs; and

  • Monitor in home service costs to assure that costs do not exceed the cost of nursing home placement.

Other Medicaid Services:

Targeted Case Management - Medicaid clients sixty (60) years of age or older who have limited functional capabilities and need assistance with the coordination of multiple services and/or resources may be eligible for this service. Case management services will assist Medicaid recipients in gaining access to needed medical, social, educational and other services.

Personal Care - A Medicaid client may receive these services if he/she requires assistance with at least two activities of daily living tasks. Services may include assistance with basic personal care, meal preparation, essential household activities and transportation.

Home Health - Medicaid clients may receive assistance with periodic nursing care to preserve life or prevent or delay the necessity of inpatient care. These services include "part-time" or "intermittent" care provided by a registered professional nurse, licensed practical nurse, student nurse or home health aide.

Hospice - Terminally ill Medicaid clients may receive a continuum of care services under this category. Services may include routine home care, continuous home care, inpatient respite care and general inpatient care.

Transportation - Medicaid clients may receive assistance with obtaining transportation to a medical facility. This may include ambulance services.
