Type 1 : Glipizide for bolus???

A friend diagnosed LADA in Jan has been on Amaryl until 2 weeks ago when she added insulin for basal.  I have been taking Starlix for bolus and she also needs a bolus.  She tried the Starlix and it made her sick.  Now she is preparing to take Glipizide for a bolus.  I thought that was about the same as Amaryl and would not be a candidate for a bolus.  Does anyone know what the time of action is for Glipizide and is it conceivable that this can act as a bolus.

If your friend was LADA (latent autoimmune diabetes of the adult), she would not be able to concieveably take oral diabetes medications as they are more towards the type 2 diabetic (glucophage/metformin, actos, can be taken by type 1's).
Glipizide is a medication that forces the beta cells of the pancreas to produce more insulin. In LADA, the beta cells slowly, die off as in type 1 (takes 2-4 years or so vs normal type 1 when it is weeks/months).
So No she would not be able to take glipizide, but the peak plasma level is in 2-3 days.. therefore can NOT be used as a "bolus" medication.

LADA is essentially slow onset type 1, and using insulin all the way has been proven to extend the life of the remaining beta cells. Therefore, I would suggest MDI or insulin pump therapy in the case of LADA.

Glipizide is a sulforanyea.(sp?) It is used to cause the Pancreas to produce more Insulin. It is usually taken once a day with the first main meal.. It is not used as a bolus. Also LADA's depending on the subset can be on Oral Medications for up to several years. It depends on the subset how fast the person becomes insulin dependent.
