I think all of us 788 owners have had this problem at one time of the other.
What worked for me is the following:
Remove the set screw from the left side and the thumb screw assembly from the right. Use a product like GooGone to completely remove all oil from the ends of both pieces. Lightly sand them and reinstall making sure that the blade will end up in the center of the holder. Might even do this to the top as well.
