"There's an online catalogue store, Dr. Leonard's that has lots of these things like Walt's sock helpers and the reach extender. 
I had one of those once, but it must've been a cheapo version because it broke fairly soon.  They advertise one on television that looks sturdier though.  There's a web site called As Seen On TV that probably has it.  
A former boyfriend of mine has MS and had a lot of
difficulty putting his shoes on.  I found him a long
metal shaft with a shoe horn on the other end, and it
had a heavy spring attaching it to the shaft so it
would curve under the foot.  It was about 30 inches
long, so he didn't have to bend over to use it, and it
worked great.  I seem to recall buying it in a luggage
store that sold leather goods and other accessories.  
One thing I can't live without is my back scratcher. 
It's made of bamboo, and I got it from a site called
Asian Ideas in New York.
